Care Coordination Specialist:

The Care Coordination Specialist is responsible for delivering continuous support services aimed at meeting the ongoing needs of clients and their families, encompassing both healthcare and mental health aspects.

This role aids clients and their families in accessing community resources, pinpointing available support, and overcoming obstacles to care.

Key Responsibilities:
Conducts continuous service coordination for individuals of all ages in alignment with established care plans.

Handles initial crisis inquiries and assesses needs to determine appropriate resources.


Employs recognized best practices to engage both formal and informal support systems for clients, including the planning for transitions, discharges, and aftercare related to mental health services, while fostering a collaborative, strengths-based approach to teamwork and shared leadership.


Facilitates the integration of mental health services with various organizations, such as Child Protective Services, Probation and Parole, Developmental Disabilities Services, educational institutions, and other service providers.


Required Qualifications:
Bachelor's Degree in a relevant behavioral health discipline is preferred; a combination of education and experience may be acceptable.

Experience in a mental health care environment is essential.

Possession of a Level 1 Fingerprint Clearance Card.

Verification of educational background, work history, and references.

Must meet driving criteria as per organizational guidelines.

Valid Arizona Driver's License and current Auto Insurance are required.
