Care Coordination Specialist: The Care Coordination Specialist plays a crucial role in delivering continuous care-coordination services aimed at addressing the ongoing needs of clients and their families, encompassing both primary healthcare and mental health support. This role is instrumental in aiding clients and families to access community resources, identify necessary support, and overcome obstacles.

Key Responsibilities:

  • Facilitates the ongoing coordination of services for both children and adults in alignment with their treatment plans.
  • Responds to initial crisis inquiries, assessing needs and identifying essential resources.
  • Employs established best practices to engage both formal and informal support systems for clients, including the transition, discharge, and aftercare planning for mental health services, while fostering a positive, strengths-based approach to teamwork and shared leadership.
  • Coordinates mental health services with various agencies, including the Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), Arizona Long Term Care, educational institutions, and other service providers.

Qualifications:

  • Bachelor's Degree in a behavioral health discipline is preferred; a combination of relevant experience and education may be considered.
  • Experience in a behavioral health environment is essential.
  • Possession of a Level 1 Fingerprint Clearance Card.
  • Verification of educational background, employment history, and references.
  • Must meet MVR criteria in accordance with JFCS driving policies.
  • Valid Arizona Driver's License and current auto insurance are required.
