Qualifications:
Education

Associate's Degree in Nursing is required.

Experience

A minimum of 2 years of nursing experience is required.

At least 1 year in an inpatient or outpatient clinical environment is required.

Licenses and Certifications

Registered Nurse (RN) - State Licensure and/or Compact State Licensure is required.

Basic Life Support (BCLS) certification is required.

Key Responsibilities:

Job Summary

This role provides a cohesive approach to case coordination by undertaking various responsibilities to ensure optimal patient care.

Evaluates medical records and collaborates with healthcare providers, patients, families, and other team members, including case management, to formulate a practical treatment plan.

Offers guidance and appropriate resources to patients and families by assessing their social, financial, and emotional needs. Works in partnership with relevant staff to coordinate consultations, referrals, initiation and continuation of home care services, and durable medical equipment orders.

Acts as a patient advocate throughout the care continuum and serves as a resource for physicians, patients, and families to facilitate effective communication.

Collaborates with patients, families, and caregivers to ensure care is coordinated across the healthcare continuum by establishing strong relationships, providing resources based on assessed needs, delivering education, and facilitating all necessary follow-up care.


In collaboration with physicians and service line leadership, develops care standards, practice guidelines, and diagnosis-based protocols, ensuring that patients receive quality care through these methodologies.

Coordinates patient referrals to other specialists and physicians.

Proactively identifies issues and suggests necessary adjustments as needed.

Ensures that all tests are conducted promptly, results are reviewed by the physician, and patients are informed of their test outcomes.


Influences optimal patient quality outcomes, promotes efficient resource utilization within the network, and ultimately enhances the patient experience while fostering high patient satisfaction.

Communicates effectively and professionally through all channels while maintaining patient confidentiality in accordance with HIPAA regulations. Documents relevant information in the patient's permanent medical record.
