Key Responsibilities:
Deliver high-quality primary care services to geriatric patients, emphasizing healthy aging and the management of chronic illnesses.
Conduct detailed assessments, perform physical examinations, and order as well as interpret diagnostic tests.
Formulate and execute personalized care plans based on individual patient requirements, which include medication management, preventive screenings, and health promotion initiatives.
Collaborate with multidisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure holistic and patient-centered care.

Assist in the coordination and provision of hospice and palliative care services, focusing on pain management, symptom relief, and emotional support for patients and their families.

Provide education to patients and their families regarding disease management, treatment alternatives, and end-of-life care considerations.

Qualifications:
Current and unrestricted Nurse Practitioner license in Arizona.
A minimum of 2 years of experience as a Nurse Practitioner, particularly in geriatrics, hospice, or palliative care.
In-depth knowledge of geriatric healthcare, chronic disease management, and principles of end-of-life care.
Strong clinical assessment and diagnostic capabilities.
Ability to work effectively within an interdisciplinary team setting.
Outstanding communication and interpersonal skills, demonstrating a compassionate and empathetic approach to patient care.
Willingness to travel within the Phoenix area.
