Responsibilities
The Healthcare Coordination Specialist - LPN serves as a vital link, promoting coordination, communication, and collaboration among Brookdale teams, residents, families, and healthcare providers within Brookdale's HealthPlus communities. They maintain open and collaborative communication channels, adapting to the evolving healthcare needs of the residents. Working closely with primary and specialty care providers to develop resident-specific care plans, ensuring that medically necessary services align with established care strategies. Acting as a resident advocate, they actively contribute to identifying and enhancing service delivery, closing care gaps, and understanding essential interventions and care protocols related to resident treatment.
- Assess the current population's medical diagnoses to identify care requirements. Assist in evaluating the health, functional, and psychosocial status of residents.
- Coordinate services necessary to promote optimal wellness and manage chronic conditions.
- Collaborate with hospice, palliative, and home health services/therapy as needed.
- Engage in collaborative care reviews (CCR) and care conferences.
- Educate and clarify Brookdale's care pathways according to individual diagnoses and healthcare needs. Provide advanced care planning education to residents and families/responsible parties. Ensure each resident has a completed Advanced Care Plan, inclusive of care goals. Communicate with healthcare providers to ensure care delivery aligns with the resident's Advanced Care Plan(s).
- Provide resources for residents and families to make informed decisions about meeting healthcare needs and advocate for the benefits of residing and receiving care and services in the community. This includes informing them about opportunities to enhance care and benefits through specialized plans designed for residents with chronic medical conditions.
- Apply quality improvement methodologies to analyze, enhance, and manage outcomes, aiming to achieve quality measure objectives. Collaborate with HWD and/or RN Care Manager to ensure service plan compliance while working with providers to achieve value-based outcomes for residents. This involves coordinating and communicating care for each resident.
- Promptly relay information to the relevant care provider regarding changes in residents' health/well-being, personal needs, risk management issues, customer service matters, and family or external healthcare provider concerns.
- Implement quality performance measures and assist with team training and education to ensure quality outcomes for residents. This requires effective communication and collaboration with providers and other healthcare entities, such as managed care coordinators, insurance providers, and external healthcare partners responsible for coordinating resources and supplies relevant to resident needs.
- Complete documentation as required by Brookdale or state regulatory agencies.

- Graduate from an accredited LPN program and passed the state licensing exam.

- Experience working in skilled nursing facilities and home health environments is a plus.
