Care Coordination Specialist

Care Coordination Specialist: The Care Coordination Specialist is responsible for delivering continuous support services aimed at addressing the ongoing needs of clients and their families, encompassing both primary healthcare and mental health services. This role aids clients and their families in securing community resources, identifying available support, and overcoming obstacles.
Key Responsibilities:
- Facilitates ongoing service coordination for children and adults in alignment with the established treatment plan.
- Addresses initial crisis situations by assessing needs and identifying necessary resources.
- Employs recognized best practices to engage both formal and informal support systems for clients, including the transition, discharge, and aftercare planning of mental health services, while fostering a collaborative, strengths-based approach to teamwork and shared leadership.
- Coordinates mental health services with various agencies, including the Department of Child Safety, Probation/Parole Departments, Division of Developmental Disabilities, Arizona Long Term Care, educational institutions, and other service providers.
Qualifications:
- Bachelor's Degree in a behavioral health discipline is preferred; a combination of relevant experience and education may be considered.
- Experience in a behavioral health environment is essential.
- Possession of a Level 1 Fingerprint Clearance Card.
- Verification of educational background, employment history, and references.
- Must meet MVR criteria in accordance with driving guidelines.
- Valid Arizona Driver's License and Auto Insurance are required.
