Key Responsibilities- Collaborate with crisis therapists to develop and implement individualized treatment plans that address the unique needs of each client.
- Conduct thorough assessments of client needs and identify gaps in service delivery to ensure seamless care coordination.
- Actively coordinate care with facility staff and other stakeholders to ensure timely and effective interventions.
- Maintain accurate and detailed records of client interactions and service delivery, including documentation of treatment plans, progress notes, and discharge planning.
- Participate in ongoing education and training to maintain and improve competency in crisis intervention and behavioral health services.
- Associate's degree in behavioral health or related field.
- At least 2 years of experience in behavioral health, with a focus on crisis intervention and care coordination.
- Knowledge of Title 36 involuntary commitment process and Maricopa County Regional Behavioral Health Authority (RBHA) system.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
