Clinical Pharmacy Specialist

The Department of Veterans Affairs is seeking a highly skilled Clinical Pharmacy Specialist to join our team. As a Clinical Pharmacy Specialist, you will play a critical role in providing exceptional patient care and contributing to the development of our pharmacy services.
Key Responsibilities- Provide direct and indirect patient care, including medication therapy monitoring, formulary management, and staff education.
- Collaborate with healthcare providers to optimize medication use and improve patient health outcomes.
- Develop and implement pharmaceutical care plans, including documentation in electronic health records.
- Order and review laboratory tests and diagnostic studies to support patient care.
- Participate in medication reconciliation and evaluate appropriateness of medications during transitions of care.
- Monitor and report drug errors, adverse drug reactions, allergies, and patient compliance issues.
- Support effective drug usage criteria and ensure appropriate utilization of the VHA formulary.
- Collaborate with physicians and other healthcare professionals in the design and implementation of drug research protocols.
- Provide in-service education programs to develop pharmacy staff and educate other healthcare disciplines.
- Evaluate physician orders for appropriateness based on patient medication profiles and input information into computer systems.
- Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
- Full, current, and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
- English language proficiency.
- At least 1 year of experience equivalent to the next lower grade level.
- Completion of a Pharmacy Practice or Specialty Residency with a substantial component (>50%) of patient care activities in pharmacotherapy.
- Three (3) years of practice experience with a substantial component (>50%) of patient care activities in pharmacotherapy.
This position requires standing (4 hours), walking (4 hours), repeated bending (4 hours), twisting, and carrying/lifting moderately heavy items (lbs). A driver's license and physical exam are required to confirm ability to operate a government vehicle.
