Care Coordinator Role at Catholic Charities
We are seeking a skilled Care Coordinator to join our team at Catholic Charities. As a Care Coordinator, you will be responsible for
* Obtaining written participant consent through progressive outreach
* Developing care plans through case conferencing and consultation with providers
* Conducting home visits and monitoring member health status
* Maintaining case records and documentation
**Requirements:**
* 4-year Bachelor's Degree in public health, human services, or related field
* Preferred licensed level clinician with prior experience in behavioral health or care management
* NYS licensure and current registration as a Registered Nurse
