Care Coordinator

Catholic Charities of the Diocese of Albany is seeking a dedicated and compassionate Care Coordinator to join our team in the Schenectady Office. As a Care Coordinator, you will play a vital role in supporting individuals and families in need, providing guidance and resources to help them achieve their goals.
Key Responsibilities:
- Develop and implement care plans in collaboration with participants, providers, and other stakeholders.
- Conduct home visits and monitor participant health status, access to services, and quality of received services.
- Maintain accurate and up-to-date case records and documentation.
- Collaborate with other providers and agencies to ensure comprehensive care and support.
Requirements:
- Bachelor's degree in public health, human services, or a related field.
- Preferred: licensed level clinician with prior experience in behavioral health or care management.
- NYS licensure and current registration as a Registered Nurse.
