About the Role

We are seeking a highly skilled Admission Coordinator to join our team at Heartland Hospice. As a key member of our admissions team, you will be responsible for ensuring the timely and efficient admission of patients into our program.

Key Responsibilities
  • Contacting Patients and Families: You will be responsible for contacting patients and their families within 1 hour of referral to coordinate their admission into our program.
  • Processing Referrals: You will process referrals by obtaining necessary admission and insurance information to ensure timely and accurate admission of patients.
  • Marketing and Outreach: You will participate in marketing efforts by developing and maintaining relationships with referral sources, identifying new opportunities for growth, and assisting in the development of strategies to increase site and company growth.
  • Reimbursement and Verification: You will ensure maximum reimbursement through initial payer source verification, benefits, and eligibility verification, as well as accurate data entry into our computer system.
  • Insurance and Admission Information: You will provide accurate insurance and admission information to admission staff to ensure proper disclosure of rights and responsibilities, financial liabilities, and benefit plan details to patients and their families.
  • Documentation and Billing: You will provide billing documentation to the Billing Coordinator as needed for final confirmation prior to admission and/or submission of bill.
Requirements
  • Education and Experience: High school diploma or equivalent with customer service experience required. Hospice or medical office experience preferred.
  • Skills and Qualifications: Strong communication, organization, negotiation, and public relation skills. Proficient with PC and MS suite of products.
  • Certifications and Licenses: LPN/LV certification or Bachelor's degree in business, social work, psychology, or nursing, or equivalent business experience preferred.
