Role Summary:
The Manager of Case Management and Social Work Services is integral to the daily functioning of the department. This role involves supporting the recruitment and oversight of all personnel within the Case Management and Social Work Services team.

It is essential to effectively evaluate and manage factors that contribute to minimizing length of stay (LOS) and optimizing cost per case through efficient resource allocation.


The Manager will oversee daily departmental operations and provide leadership in the absence of the department head.

This position ensures that clinical documentation accurately represents resource management and the severity of the patient population's illnesses.


The Manager serves as a vital link between compliance, clinical management, patient progression, utilization data collection and reporting, community outreach, and readmission prevention strategies.


Qualifications:
Current NYS RN Licensure or a master's degree in social work with active licensure.

Experience in Utilization Review, Discharge Planning, Case Management, or post-acute discharge planning is required.

Eligibility for PRI or Screen certification is necessary.

Certification Requirements:
Certified Case Manager status is preferred.

A proven history of self-directed, independent practice with the ability to coordinate and collaborate effectively with various processes and individuals is essential.

1-2 years of supervisory experience is preferred.

Physical Requirements:


Ability to push/pull, lift/move 15 lbs., and perform moderately challenging manual tasks such as typing, writing, and filing for extended periods. Must be capable of performing tasks requiring hand-eye coordination, including data entry and operating office equipment.

Mobility requirements may include the ability to remain stationary at a workstation for extended periods, as well as the capacity to squat or move for reasonable distances.

Communication skills must include comprehension of spoken English and the ability to communicate effectively in both written and verbal forms.


Key Responsibilities:
Direct management of daily operations and functions of the Case Management and Social Work Services department.

Coordinate staff scheduling within the department.

Assist with payroll processing for the Case Management and Social Work Services team.

Support the completion of annual performance evaluations for departmental staff.

Oversee orientation and ongoing education for team members.

Contribute to the development of policies, protocols, and procedures.

Participate in departmental and organizational meetings and training sessions as required.

Serve as a role model for exemplary case management practices and performance standards.

Ensure adherence to all human resources protocols, including hiring, disciplinary actions, evaluations, and grievance procedures.

Comply with hospital policies and procedures.

Engage in the administrative aspects of Healthcare Resource Management and the hospital-wide Quality Improvement program.

Provide guidance, support, and an environment conducive to staff and patient responsibilities.

Assist the department head with HR-related tasks.

Utilize data analytics to focus efforts on key performance indicators, including care progression, length of stay, and quality metrics.

Assume the responsibilities of the department head in their absence.

Perform additional duties as assigned.

Compensation:
Salary range is $31.50-$47.26 per hour, based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be compensated within the base salary equivalent of the stated hourly rates. The pay range may vary based on location.
