Position Summary:

The Case Manager will employ an integrated approach to case management, working collaboratively with an interdisciplinary team to develop personalized care plans. This position requires a proactive engagement with members to identify their medical and psychosocial needs, providing education, resources, and healthcare coaching. The ultimate goal is to facilitate access to quality care, ensuring members can navigate the healthcare system effectively, including understanding their benefits.

Key Responsibilities:
  • Conduct thorough assessments to determine the health needs of members.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Provide ongoing monitoring and support to ensure optimal health outcomes.
  • Educate members about their healthcare options and benefits.
  • Coordinate with healthcare providers to ensure continuity of care.
  • Maintain accurate documentation of member interactions and care plans.

Qualifications:
  • Current licensure as a Registered Nurse in New York State is mandatory.
  • Possession of Vermont licensure is advantageous.
  • A Bachelor's degree in nursing is preferred.
  • Certification in Case Management is desirable.
  • Minimum of three years of clinical and case management experience is required.
  • Familiarity with Medicaid and Medicare programs is strongly preferred.

  • Exceptional communication skills, both verbal and written.
  • Strong clinical knowledge and understanding of population health management.
  • Detail-oriented with excellent organizational abilities.
  • Ability to work effectively in a collaborative team environment.
