Community Outreach Case Manager
The role of the Community Outreach Case Manager involves:
- Connecting with individuals identified by outreach initiatives and evaluating their eligibility and needs based on personal goals. Facilitating referrals to community resources and conducting ongoing assessments to address persistent challenges that impact care delivery, community stability, and health improvement.
- Participating in scheduled outreach meetings and maintaining communication with team members as necessary.
- Delivering intensive case management services to clients until they are linked with permanent care management agencies.
- Ensuring comprehensive transitional care from inpatient settings to other environments, linking individuals with essential services upon discharge or transition.
- Coordinating with other service providers to facilitate information exchange and identify additional care needs. Monitoring service delivery and engagement, advocating for further services as required.
- Providing transportation for clients when needed, utilizing agency vehicles. Employees must have a valid driver's license and be approved to operate agency and personal vehicles for work purposes, ensuring safe driving practices.
Qualifications
Essential skills and qualifications for the Community Outreach Case Manager include:
- A minimum of one year of experience in case management, encompassing assessment of individual needs, care planning, implementation, and regular review, along with knowledge of benefit entitlements and connections to community services.
- Ability to work effectively as part of an interdisciplinary team within the mental health care system.
- Understanding of mental health issues, substance use, physical health, and homelessness.
- A valid and insurable driver's license, with personal vehicle availability for business use.
Compensation and Work Schedule
The Community Outreach Case Manager position offers:
- Hourly wage of $21.87, equating to an annual salary of $45,489.
- A full-time schedule of 40 hours per week, with flexibility to work four 10-hour days. One weekend day is required. Comprehensive benefits, staff training, generous paid time off, and a 30-minute paid lunch break are included.
