Job Overview

Position: Outreach Case Manager

Key Responsibilities:

  • Connect with individuals referred by the Outreach HUB, evaluating their eligibility and needs based on personal goals. Facilitate referrals to community resources and conduct ongoing assessments to address persistent challenges impacting care delivery, community stability, and health improvement.
  • Participate in scheduled Outreach HUB meetings and maintain regular communication with fellow HUB members as required.
  • Deliver intensive case management services weekly until individuals are linked with permanent Health Home Care Management agencies.
  • Ensure comprehensive transitional care from inpatient settings to other environments, linking individuals with essential services upon discharge or transition to different service providers.
  • Collaborate with other care providers to ensure effective information exchange and identify additional care needs. Monitor service delivery and maintain ongoing engagement, advocating for further services as necessary.
  • Provide transportation for individuals when required, utilizing agency vehicles. Employees must possess a valid NYS driver's license and be approved by the agency's insurance carrier to drive agency and personal vehicles for business purposes. Safe driving practices and medical fitness to drive are essential. Willingness to use personal vehicles for community outreach is expected unless transporting an individual.

Qualifications:

To be considered for the Outreach Case Manager role, candidates should possess:

  • A minimum of one year of relevant experience in case management, encompassing assessment of individual needs, care planning, implementation of care strategies, and regular reviews alongside benefit entitlements and community provider linkages.
  • Proven ability to work effectively as part of an interdisciplinary team within the mental health care system.
  • Comprehensive knowledge of mental health issues, substance use disorders, physical health, and homelessness.
  • A valid and insurable NYS driver's license, with access to a personal vehicle for business purposes. Agency vehicles are available for client transportation.

Compensation and Benefits:

The Outreach Case Manager position offers:

  • Hourly wage of $21.87, equating to an annual salary of $45,489.
  • A standard workweek of 40 hours, with flexible scheduling options including four 10-hour days. Weekend availability is required. Excellent benefits, comprehensive staff training, generous paid time off, and a 30-minute paid lunch break are included.
