Position Summary: The Case Manager plays a vital role in supporting individuals and families in need of social services. This position is crucial in assisting clients to navigate their challenges and achieve their personal goals.

Key Responsibilities:

  • Provide assistance to individuals and families seeking social service support.
  • Conduct interviews and assessments to evaluate applicants for services, develop service plans, and help clients implement these plans.
  • Guide tenants in achieving their objectives by identifying community resources and making appropriate referrals.
  • Assess clients' living conditions through home visits.
  • Support tenants in overcoming obstacles to their goals and empower them to advocate for themselves, promoting self-sufficiency.
  • Facilitate group sessions and workshops aimed at enhancing independent living skills.
  • Monitor tenants' progress towards their goals through regular communication and document updates.
  • Act as a liaison and advocate for tenants with external organizations regarding education, healthcare, housing, and legal matters.

Qualifications:

  • A Bachelor's degree (e.g., B.A., B.S.W.) along with two years of relevant experience or equivalent qualifications.

Compensation: The annual salary ranges from $40,000 to $45,000, depending on various factors such as experience and education.

Employment Status: This is a full-time position (35 hours per week).
