Position: Housing Support Specialist

Unit: Scattered Site Housing

Reports to: Program Supervisor

Status: Full Time, Regular, Non Exempt

Salary Range: $45,000 - $55,000

Work Schedule: Monday - Friday, 9:00 AM - 5:00 PM (flexibility required)

Position Summary:

The Housing Support Specialist will evaluate, counsel, and connect individuals experiencing homelessness or those with a history of incarceration, mental health challenges, substance use, and/or HIV/AIDS to essential transitional and permanent housing solutions. This role involves providing continuous supportive services to help participants sustain their housing.

Key Responsibilities:
  • Conduct initial assessments and interviews for newly released individuals and those eligible for housing assistance;
  • Develop personalized treatment plans for each participant, ensuring all services are documented promptly to maintain up-to-date records;
  • Provide crisis intervention as necessary;
  • Establish and manage suitable housing options for participants;
  • Evaluate the need for temporary rental assistance and moving costs;
  • Collaborate with various agencies to ensure timely access to entitlements for participants;
  • Assist participants in gathering necessary documentation for benefits applications and submit applications to relevant agencies;
  • Act as a liaison with benefits administration representatives;
  • Mediate landlord-tenant issues on behalf of participants;
  • Offer ongoing counseling and support to help participants achieve their treatment goals;
  • Conduct home visits, hospital visits, and community-based visits as required;
  • Perform annual inspections of apartments to ensure a safe living environment;
  • Coordinate with Property Management for necessary repairs;
  • Accompany participants to appointments when needed;
  • Facilitate case conferences with community service providers as necessary;
  • Provide support for participants outside of the regular caseload when required.
Qualifications:

Education: Associate's Degree in Human Services or five years of relevant experience; a High School Diploma with eight years of experience, or a Bachelor's Degree in Human Services with at least two years of experience with the target population is preferred. CASAC or CASAC-T certification with two years of experience is also preferred.

Experience: Candidates should be capable of managing a maximum caseload of 15 participants and must demonstrate the ability to assess needs collaboratively, create and implement service plans, monitor progress, and evaluate services provided.

Technical Skills: Proficiency in Microsoft Excel, Word, and Outlook, along with experience in accounting software and basic knowledge of PowerPoint.

Communication Skills: Excellent writing, editing, and verbal communication abilities are essential.

Interpersonal Skills: Strong interpersonal skills with the ability to engage effectively with diverse individuals in a culturally sensitive manner.

Travel Requirements: Must be physically able to travel throughout the area using public transportation as needed.

Physical Demands: Candidates must be able to perform essential duties satisfactorily, including climbing stairs for home visits when necessary.
