As a Registered Nurse Care Coordinator, you will work in partnership with inter-professional teams throughout the care continuum to facilitate and guarantee effective transitional care management.

Key Responsibilities:
Facilitates inter-professional collaboration throughout the acute care episode and ensures effective handoff to the next level of care or community to maintain continuity of care.
Proactively evaluates, identifies, and addresses potential barriers to patients' transitions of care.
Collaborates with the patient and/or their care partners to create a detailed, individualized transitional care plan. Participation and leadership in patient rounding activities, huddles, and committee meetings are expected, as appropriate.
Education Requirements:
Graduation from an accredited nursing program.
BSN is preferred.
Certifications:
Current State Licensed Registered Nurse (RN).
Certification from an approved entity is preferred within one year of eligibility, e.g., CCTM, ACM, CCM. American Heart Association.
Basic Life Support (BLS) certification through American Heart Association (AHA)
Competencies (Complexity of Work):
Requires critical thinking skills, decisive judgment, and the ability to work independently.
Must be capable of functioning in a high-pressure environment and taking appropriate actions.
Work Experience:
A minimum of three years of recent clinical RN experience is required. At least two years of recent experience in Utilization Review, Quality, or Care Management is preferred.
Familiarity with acute care regulatory/accreditation standards is preferred. Experience in discharge planning and/or Quality Improvement is also preferred.
Physical Requirements:
Requires normal or corrected hearing and vision within the normal range.
Ability to perform work in a stationary position for extended periods.
Ability to communicate and exchange accurate information.
Ability to manage physical records, including retrieving and filing. Proficiency in operating a computer and other office equipment.
Ability to travel throughout the healthcare system.
In some locations, the ability to lift up to 10 pounds may be required.
Personal Protective Equipment:
Adheres to Standard Precautions by utilizing personal protective equipment as necessary for procedures.
