Role Purpose:
In this leadership position, you will oversee the care coordination team dedicated to serving long-term care members. Your responsibilities will include:
- Supervising daily operations and addressing escalations related to care management.
- Ensuring the delivery of high-quality care management services.
- Collaborating with both members and providers to resolve care-related issues.
Qualifications:
To be considered for this role, candidates must possess:
- A Bachelor's degree.
- A minimum of 4 years of relevant experience in care management or a related field.
- Equivalent experience that demonstrates the necessary knowledge and skills for this position.
