Job Description

We are seeking a skilled Registered Nurse to join our team as a Care Manager, Value-Based Programs. The successful candidate will have a strong clinical background and experience in value-based care programs, quality, or care transitions.

Responsibilities

  1. ">
  2. Collaborate with the Executive Director of Care Management & Director of Quality to create and implement strategic plans for value-based care management programs.">
  3. Coordinate complex patient care throughout the continuum of care, working with clinical teams and case management inside and outside of Stamford Health.">
  4. Track and manage value-based care management program efficiency, cost, and utilization measures.">
  5. Participate in the development of budgets, health utilization measures, and financial measures.">
  6. Analyze payor and internal data to target high-risk, high-cost, and high-utilizer patients of Stamford Health Medical Group and Stamford Hospital.">

Qualifications

  • ">
  • Bachelor's degree in Nursing (BSN) with licensure in the state of Connecticut required.">
  • Strong analytical skills with the ability to interpret data and create strategic plans based on cost, utilization, and other quality indicators.">
  • Experience in value-based care programs, quality, or care transitions preferred.">
  • Certification in Case Management or Clinical Specialty highly desirable.">

Estimated Salary

The estimated salary for this role is $90,000 - $110,000 per year, depending on experience.
