MDS Care Coordination Specialist

We are seeking a skilled Medical Coordinator RN or LPN to join our team at Ark healthcare & Rehabilitation at St. Camillus Center. As a key member of our interdisciplinary team, you will play a crucial role in ensuring accurate reimbursement and maximizing benefits for our residents.
Your responsibilities will include:
- Coordinating the completion of the MDS and other required assessments
- Tracking and managing case-mix documents to ensure timely submission
- Collaborating with IDT members to identify areas for improvement and develop strategies for increased efficiency
To succeed in this role, you will need:
- An unencumbered State of CT RN or LPN License
- Experience with electronic medical records systems, preferably Point Click Care
- Strong analytical and problem-solving skills
Our company offers a competitive salary range of $75,000 - $95,000 per year, commensurate with experience.
