We are seeking an experienced Part Time LPN Care Coordinator to join our team at The Residence at Summer Street in Stamford, CT. As a key member of our Resident Care Team, you will be responsible for assisting the Resident Care Director with the management of the community.

  • Promote the mission and core values of LCB Senior Living to residents, visitors, co-workers, and the surrounding community.
  • Growing community occupancy by representing and interacting positively with potential residents, family members, and professionals.
  • Assist with the evaluation of resident needs and the development and revision of service plans as per state regulatory requirements.
Required Skills and Qualifications:Education:

A high school diploma and graduation from an accredited Nursing Program are required.Experience:

At least two years of experience in a geriatric setting and previous health experience in hospital and/or community setting are preferred.Certificates/Licenses:

A current RN or LPN License in the state of employment is required.
