Job Title: Healthcare Professional - Patient Care Specialist

We are seeking a dedicated and compassionate Healthcare Professional - Patient Care Specialist to join our team at Patient Care. As a key member of our home health care team, you will play a vital role in providing high-quality patient care and support.

About the Role:

  • You will work closely with patients and their families to understand their unique needs and develop personalized care plans that meet their goals and preferences.
  • As a skilled nurse, you will provide direct patient care, including administering medications, wound care, and other medical treatments as prescribed by physicians.
  • In collaboration with other healthcare professionals, you will assess patient conditions, identify potential risks, and implement interventions to prevent complications and promote optimal outcomes.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Provide direct patient care, including medication administration, wound care, and other medical treatments as prescribed by physicians.
  • Collaborate with interdisciplinary teams to coordinate patient care and services.

Requirements:

  • RN licensure in the state of practice.
  • Current CPR certification.
  • Reliable transportation and valid driver's license.

Salary Range: $65,000 - $85,000 per year, depending on experience.
