Patient Care Coordinator

The Patient Care Coordinator will be responsible for ensuring the complete and timely patient flow process across different sites in Stamford. The estimated salary for this position is $55,000-$70,000 per year, based on industry standards and location.
Key Responsibilities:
- Coordinate patient appointments and schedules
- Update patient information in Epic (EMR system)
- Manage in-basket messages
- Take patient history and vital signs
You will work closely with providers, nurses, and other front office staff to ensure seamless communication and coordination of care.
Required Qualifications:
- Graduated from an accredited Medical Assistant Program
- Professional Medical Assistant Certification obtained through a reputable agency
- Experience in a clinical setting, preferably with Electronic Medical Records (EMR) systems
- CPR certification required upon hire and recertification bi-annually
