The Patient Care Coordinator will be responsible for ensuring the complete and timely patient flow process across different sites in Stamford. The estimated salary for this position is $55,000-$70,000 per year, based on industry standards and location.

Key Responsibilities:

  • Coordinate patient appointments and schedules
  • Update patient information in Epic (EMR system)
  • Manage in-basket messages
  • Take patient history and vital signs

You will work closely with providers, nurses, and other front office staff to ensure seamless communication and coordination of care.

Required Qualifications:

  • Graduated from an accredited Medical Assistant Program
  • Professional Medical Assistant Certification obtained through a reputable agency
  • Experience in a clinical setting, preferably with Electronic Medical Records (EMR) systems
  • CPR certification required upon hire and recertification bi-annually
