POSITION OVERVIEW:

The individual in this role will serve as a vital member of an interdisciplinary team, collaborating with both professional and non-professional staff within a rehabilitation treatment framework.

The Case Manager will undertake a variety of supportive responsibilities, including aiding Veterans in defining their recovery objectives, acquiring and practicing new skills, tracking their progress, assisting in their treatment journey, demonstrating effective coping strategies, and empowering them to advocate for their own access to essential services.

KEY RESPONSIBILITIES:
  • Collaborate with Mental Health Counselors to deliver assessment, counseling, reintegration, and rehabilitation services to participants.
  • Evaluate the needs, conditions, and aspirations of assigned participants and provide comprehensive case management services.
  • Implement all services within the Zero Suicide framework.
  • Act as an advocate for participants to enhance their access to critical services.
  • Engage in case conferences involving assigned participants.
  • Complete and review all documentation related to clinical services in compliance with regulatory standards and organizational policies.
  • Conduct community outreach to identify and engage eligible individuals and their families.
  • Coordinate emergency clinical services for Veterans identified as at risk of suicide.
  • In collaboration with Mental Health Counselors, develop, monitor, and document individualized suicide prevention plans.
  • Provide referrals and assist participants in accessing necessary services based on their identified needs and goals.
  • Support participants in obtaining and coordinating VA benefits and other public health assistance.
  • Maintain regular contact with participants to ensure resource linkages and facilitate warm hand-offs to service providers.
  • Embody the mission, vision, and guiding principles of WestCare Foundation.
  • Perform additional duties as assigned.
QUALIFICATIONS:

To excel in this position, candidates must demonstrate the ability to perform the outlined responsibilities effectively. The following qualifications are essential:

  • Proven experience in counseling, therapy, or social work is preferred.
  • Experience working with individuals facing mental health and substance abuse challenges is required.
  • A degree in Counseling, Social Work, Psychology, or a related field is preferred.
  • A High School Diploma or equivalent is mandatory.
  • Licensed Professional Counselor (LPC) certification is preferred.
  • Ability to exercise sound judgment and discretion.
  • Proficiency in computer use and document preparation.
  • Ability to respond effectively to crisis situations.
  • Commitment to ethical standards and confidentiality.
  • Cultural and linguistic sensitivity to the populations served.
  • Must successfully obtain and maintain a criminal background clearance.
  • Valid driver's license and ability to meet vehicle insurance requirements.
WORKING ENVIRONMENT:

The role primarily takes place in an office or treatment setting, with some outdoor activities required. The ability to de-escalate situations effectively is essential.
