About This Role

We are seeking a dedicated and compassionate Mental Health Case Manager to join our team. In this role, you will play a vital part in providing comprehensive support and guidance to individuals facing mental health challenges.

Responsibilities

  • Conduct thorough individual assessments to understand clients' needs and develop personalized treatment plans.
  • Provide direct case management services, including crisis intervention, advocacy, and resource coordination.
  • Collaborate with a multidisciplinary team of professionals to ensure holistic client care.
  • Maintain accurate and up-to-date clinical documentation in accordance with established standards.
  • Adhere to ethical guidelines and maintain confidentiality at all times.

Qualifications

A strong understanding of community mental health principles and practices is essential. You should possess excellent communication, interpersonal, and problem-solving skills.

Knowledge, Skills, and Abilities

  • Working knowledge of community mental health concepts and/or chemical dependency case management and treatment services.
  • Experience with individual assessment, treatment planning, and treatment protocols.
  • Familiarity with mental health disorders and diagnostic classifications.
  • Ability to establish positive working relationships with individuals, families, and other agencies.
  • Proficiency in preparing concise and complete clinical documentation.
  • Strong teamwork and collaboration skills.
  • Commitment to ethical conduct and confidentiality.

Physical Requirements

This role may involve operating a motor vehicle, sitting, standing, moving about the work area, climbing stairs, using a keyboard, lifting up to 15 pounds, speaking clearly, reading text, hearing normal speech levels, and working in environments that may be exposed to various conditions such as heat, cold, humidity, dust, second-hand smoke, noise, or household cleaning chemicals.
