Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Case Manager. As a Hospice Case Manager, you will be responsible for managing the care of our patients in their homes, providing emotional support and guidance to patients and their families, and collaborating with our interdisciplinary team to ensure seamless care delivery.

Responsibilities
  • Manage the care of patients in their homes, including assessing their needs, developing care plans, and coordinating with healthcare providers.
  • Provide emotional support and guidance to patients and their families, addressing their concerns and answering their questions.
  • Collaborate with our interdisciplinary team, including physicians, social workers, and other healthcare professionals, to ensure seamless care delivery.
  • Document patient information, including medical history, treatment plans, and progress notes.
  • Participate in on-call rotations, providing 24/7 support to patients and their families.
Requirements
  • Current RN licensure in the state.
  • At least 6 months of licensed experience in any setting.
  • Valid Driver's license and auto insurance.
  • CPR certification.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
