Position Overview:
The Clinical Social Services Coordinator plays a vital role in conducting evaluations, providing psychosocial support, and implementing interventions for patients and their families as part of a collaborative healthcare team.

Key Responsibilities:
This role necessitates the application of critical thinking and judgment to assess the needs of patients and families, crafting tailored care strategies.

The Clinical Social Services Coordinator's duties encompass:

  • Conducting comprehensive assessments and interventions.
  • Providing anticipatory grief counseling and care planning.
  • Facilitating end-of-life discussions and documentation.
  • Engaging in effective communication and collaboration with interdisciplinary team members.
  • Utilizing resources efficiently to support hospice patients and their families.

Qualifications:
Applicants should possess:

  • A Master of Social Work (MSW) from an accredited institution recognized by the Council on Social Work Education.
  • A current LCSW, LMFT, or LMHC license in the State of Florida.
  • A BSW or equivalent degree in Health and Human Services, with relevant healthcare experience considered.
  • Preferred experience in hospice care.
  • A minimum of two years of experience in patient care, home care, or acute care settings.
  • Strong self-direction and the ability to work independently as well as collaboratively.
  • Commitment to ongoing professional development through education and training.
  • Excellent written and verbal communication skills.
  • Regular attendance is a condition of employment.

Expectations:
The Clinical Social Services Coordinator is expected to:

  • Develop program strategies that enhance team cohesion and collaboration.
  • Work alongside various departments to foster leadership and a comprehensive approach to program development.
  • Inspire staff to embody the mission, vision, values, and commitments of Treasure Coast.
  • Stay informed on legal and compliance issues related to hospice care.

Key Result Areas:
Responsibilities include:

  • Performing clinical assessments and interventions as part of the Interdisciplinary Team.
  • Conducting suicide assessments and formulating safety plans as necessary.
  • Exhibiting professional counseling skills, including engaging patients and caregivers, reframing issues, and normalizing emotions.
  • Leading group sessions for patients and families to process grief and emotions related to death.
  • Ensuring timely and accurate documentation of psychosocial assessments, care plans, and progress notes in compliance with regulatory standards.
  • Maintaining therapeutic relationships and healthy boundaries with diverse patients and families.

Advocacy Role:
The Clinical Social Services Coordinator will also:

  • Act as an advocate for patients and families, assisting with future care planning, funeral arrangements, and financial information.
  • Respond promptly to immediate needs or crises, ensuring timely communication with supervisors.
  • Document all patient interactions and contacts within a 24-hour timeframe.
