Role Responsibilities
The RN Case Manager is responsible for:
- Providing orientation to patients and their families regarding the case management process, rehabilitation philosophy, and discharge criteria.
- Coordinating educational efforts for patients and families, fostering their involvement in care discussions.
- Implementing individualized treatment plans that align with patient preferences and needs.
- Documenting assessments and communicating any social or financial needs of patients.
- Promoting positive interactions that enhance patient satisfaction and service quality.
Qualifications Required
- Current RN licensure in the state; BSN preferred.
- A minimum of 5 years nursing experience, including 2 years in critical care or 3 years in nursing with 2 years in case management.
- Preferred certifications in case management or nursing.
- Experience with InterQual guidelines is advantageous.
- Basic Cardiac Life Support certification required within 30 days of employment.
- Associate Degree or Registered Nurse Diploma is acceptable.
