Client Care Coordinator

Position Overview:

The role of the Case Manager is to advocate for clients, empowering them through education and support.

Key Responsibilities:

  1. Oversee clients transitioning from acute care settings to home, or those requiring care coordination and assessments.
  2. Manage referrals from insurance providers.
  3. Ensure compliance with credentialing standards in healthcare systems.
  4. Utilize motivational interviewing techniques during hospital and home visits.
  5. Conduct follow-up home visits within 48 hours post-discharge when necessary.
  6. Perform assessments, including HomeMeds, and develop care plans based on healthcare agreements.
  7. Promptly follow up on care coordination efforts.
  8. Identify and facilitate client support systems, ensuring timely medical appointments after hospitalization.
  9. Establish a network of community resources and social service agencies.
  10. Maintain operational systems to support ongoing program functions.
  11. Collaborate with both internal and external teams during care transitions.
  12. Address crisis situations with appropriate and timely interventions.
  13. Comply with all relevant legal, departmental, and healthcare regulations.
  14. Foster positive community relationships and referral networks, seeking new resources.
  15. Ensure effective communication through:
  • Clear verbal and written exchanges.
  • Conflict resolution and management of unusual situations.
  • Building positive relationships and team dynamics.
  • Timely documentation and correspondence.
