The role of the Case Manager is to support, empower, and educate clients throughout their healthcare journey.
Key Responsibilities:
- Oversee clients as they transition from acute care facilities to their homes, or assist those requiring care coordination and assessments.
- Manage referrals from insurance providers.
- Ensure compliance with credentialing standards in healthcare systems.
- Utilize motivational interviewing techniques during hospital and home visits.
- Conduct follow-up home visits within 48 hours post-discharge.
- Perform assessments via home visits or telephonic communication, and develop care plans as needed.
- Promptly follow up on care coordination efforts.
- Identify and facilitate client support systems, ensuring timely medical appointments after hospitalization.
- Establish a network of community resources and social service organizations.
- Maintain operational systems to support program efficiency.
- Collaborate with team members and external partners during care transitions.
- Address urgent situations with appropriate interventions.
- Comply with all relevant legal, departmental, and healthcare regulations.
- Foster positive relationships within the community and build referral networks.
- Ensure effective communication:
- Provide clear verbal and written communication.
- Resolve conflicts and manage unique situations.
