Position Overview:
The Healthcare Support Coordinator plays a vital role in fostering a collaborative approach to healthcare and community services, addressing medical, social, developmental, behavioral, educational, and financial needs to enhance overall health and wellness.

Key Responsibilities:
- Coordinate, monitor, and assess provider services and care, including home and/or school visits, physician consultations, and therapy sessions for designated client groups.
- Conduct comprehensive assessments of client needs through detailed interviews.
- Maintain thorough documentation of all client interactions and agreements made on behalf of clients and their families.
- Evaluate and record the progress of individual therapeutic programs, making necessary adjustments as required.
- Facilitate access for clients and families to healthcare, educational, and community resources.
- Assist clients and families in developing natural support systems and connecting them with relevant networks.
- Research and familiarize oneself with community resources.
- Travel within the service area to engage with clients, families, service providers, and community partners.
- Collaborate with clients and/or families to assess, create, implement, monitor, and adjust care plans.
- Perform additional duties as assigned.

Qualifications:
- Bachelor's degree in social work, counseling, psychology, or a related healthcare field from an accredited institution, or a Bachelor's degree in a non-related field with at least one year of experience in the healthcare sector.

Working Environment:
This role operates in a dynamic, professional office setting and typically requires the use of standard office equipment such as computers and telecommunication devices. The position may involve travel for meetings and events.

Physical Requirements:
This is primarily a sedentary role, though it may involve prolonged periods of standing or sitting, bending, and using hands for filing or operating equipment. Regular communication and specific vision capabilities are essential.

Conditions of Employment:
Compliance with health screening and vaccination policies is required. A valid state driver's license is necessary.
