ROLE SUMMARY:

In the role of Care Coordinator, you will be tasked with delivering direct services to your assigned caseload, which includes conducting needs assessments, developing service plans, coordinating care, maintaining records, and providing crisis intervention. You will also support the Re-entry Task Force Coordinator in various capacities.

KEY RESPONSIBILITIES:

  • Demonstrate and promote the agency's core values in all professional interactions
  • Provide thorough Care Coordination for clients with intricate needs
  • Assist in clinical needs assessments as necessary
  • Collaborate effectively with parole officers and community partners
  • Work alongside enrolled individuals to meet established goals and benchmarks
  • Ensure all service planning obligations are fulfilled and facilitate connections to support services
  • Engage with clients and their support systems through ongoing outreach efforts
  • Maintain relationships with various support services
  • Identify and mitigate service barriers as an advocate for social change
  • Accurately and promptly execute recordkeeping, statistical tracking, and quality assurance duties
  • Support the team and coordinator by assisting with various responsibilities
  • Complete all required training for the agency and relevant authorities
  • Perform additional duties as assigned

SKILLS AND QUALIFICATIONS:

  • Proven ability to effect change and work autonomously
  • Strong clinical judgment and case management skills
  • Excellent teamwork and interpersonal communication abilities
  • Outstanding written and verbal communication skills
  • Familiarity with various software applications

EDUCATIONAL BACKGROUND:

  • Master's degree with a license or CASAC is preferred
  • A Bachelor's degree is required

PROFESSIONAL EXPERIENCE:

  • At least two years of experience working with the re-entry population

A valid Driver's License with a clean driving record and access to a personal vehicle is necessary.

SALARY RANGE: $51,874 - $66,024 annually
