We are seeking a dedicated Registered Nurse Care Coordinator to enhance our healthcare delivery at Great Lakes Integrated Network, Inc.

Position Overview:

The Registered Nurse Care Coordinator will collaborate within a multidisciplinary healthcare team in a primary care and telephonic environment. This role focuses on patient coaching and care coordination for individuals requiring assistance in navigating their healthcare journey, addressing nursing care needs, and providing follow-up after clinical events. The Care Coordinator RN will identify patients at risk and support healthcare providers in developing strategies for managing preventative care, transitions of care, and chronic disease management, utilizing established protocols and sound clinical judgment. This position is vital in promoting patient-centered care and working alongside primary care providers and medical home team members.

Key Responsibilities:

  • Assess patients for care management services and determine the appropriate level of care coordination.
  • Conduct comprehensive evaluations to identify patient risks and formulate care plans using clinical expertise.
  • Collaborate with physicians and in-house resources, including pharmacists and social workers, to create individualized care plans with measurable goals.
  • Monitor and revise care plans to reflect progress towards established objectives and self-management initiatives.
  • Engage with patients, families, and multidisciplinary teams to explore care options and community resources.
  • Support patient adherence to medical plans through collaboration with healthcare providers.
  • Oversee and guide non-clinical staff in care coordination efforts.
  • Ensure the provision of necessary home care, hospice care, and ancillary services as directed by the care team.
  • Coordinate referrals and authorizations within care management.
  • Facilitate communication between hospitals, long-term care facilities, specialists, and home health representatives.
  • Utilize data to identify high-risk patients and coordinate care effectively.
  • Identify and leverage cultural and community resources to align with patient preferences.
  • Ensure behavioral health screenings are conducted and appropriate interventions are provided.
  • Attend training and collaboration sessions as required.
  • Maintain open communication regarding patient status with healthcare providers.
  • Obtain necessary records from other healthcare providers as needed for care coordination.
  • Foster constructive relationships with internal teams and community resources.
  • Perform other job-related duties as assigned.

Qualifications:

  • Current New York State RN License.
  • Bachelor's degree in nursing preferred; an Associate degree in nursing is the minimum requirement.
  • 2-3 years of experience in acute care, rehabilitation, home care, or managed health plans.
  • Preferred: Certified Case Management (CCM) certification.
  • Preferred: Experience in Care/Case Management.

Required Knowledge:

  • Understanding of utilization management and care management principles.
  • Familiarity with health plan contracts and benefit eligibility.
  • Proficient in timely documentation within designated technology platforms.
  • Adaptability to new technologies and software.
  • Proficiency in EMR systems and basic PC skills (MS Word, Outlook, Excel).

Compensation: The salary range for this position is $80,000 - $93,000 annually, based on experience and qualifications.
