Position Overview: We are seeking a full-time Patient Navigator to collaborate within a multidisciplinary team, responding to and coordinating referrals from various sources, including governmental and non-profit organizations, hospitals, medical groups, health centers, and mental health and substance use treatment providers.


Key Responsibilities:

- Deliver enhanced patient navigation services to identify and connect eligible individuals to project offerings;
- Engage in outreach initiatives at community locations and facilitate in-reach at TTC service sites;
- Provide necessary follow-up to engage potential participants or those who may have disengaged from care.

The ideal candidate will possess a minimum of two years of experience in customer service, healthcare services, outreach, health promotion, intake coordination, or related fields. A strong understanding of substance use disorders and mental health treatment services, as well as integrative care, is essential.
