Patient Care Coordinator

We are seeking a highly skilled and compassionate Patient Navigator to join our team at JWCH Institute. The successful candidate will work closely with healthcare teams, resource providers, and key stakeholders to improve coordination of patient care and overcome barriers to cancer screening.
Key Responsibilities- Client Outreach and Engagement: Develop and implement effective outreach strategies to promote awareness of cancer screening and connect clients with necessary resources.
- Data Management: Enter data on encounters in patient navigator database and track positive cancer screening tests in a registry managed by the Quality Improvement Department.
- Central Care Coordination: Function as the central care coordinator for patients, ensuring seamless communication and coordination with healthcare teams and community resources.
- Follow-up and Referrals: Track completion of follow-up consultations and diagnostic tests, and coordinate with case managers for community referrals to address social determinants of health.
- Documentation and Reporting: Document services provided, referrals, and other relevant information in the Electronic Health Record (EHR) system.
- Transportation and Accompaniment: Coordinate transportation for screening or medical appointments, and provide accompaniment as needed.
- Emotional Support and Resource Connection: Connect clients with additional emotional support and resources, including behavioral health services and nutritional support.
- Cultural Competence and Communication: Assist clients from diverse cultural backgrounds in understanding the healthcare system and provide effective communication with healthcare teams.
- Liaison and Representation: Serve as a liaison between clients and medical staff, and represent the clinic in all matters related to healthcare services.
- Training and Skill Building: Participate in training and skill-building workshops to enhance knowledge and skills in cancer screening, diagnostics, and treatment.
- Experience and Skills: Proven experience working with clients and staff from diverse socio-economic, ethnic, and cultural backgrounds, with excellent communication and interpersonal skills.
- Healthcare Experience: Experience working in a healthcare setting, with a strong understanding of healthcare systems and processes.
- Education and Certifications: LVN or Medical Assistant experience preferred, with a current California Driver License and ability to travel to different work sites.
- Language Skills: Bilingual (English, Spanish) preferred.
