Job Overview

SUMMARY:

The Healthcare Patient Advocate (HPA) is a vital non-clinical role focused on fostering patient engagement through both telephonic and face-to-face interactions. The HPA collaborates closely with healthcare teams to promote patient-centered care and address care gaps through proactive outreach, appointment scheduling, and facilitating connections to essential resources, including transportation and interpretation services. This position requires a commitment to embodying compassionate and respectful behavior in all interactions.

KEY RESPONSIBILITIES:

Patient Outreach and Engagement: Utilize active listening and empathy to effectively communicate with patients, addressing potential barriers to appointment attendance.

Facilitating Access to Care: Coordinate with healthcare providers to ensure timely access to necessary diagnostic and wellness services. Assist patients in navigating additional resources and clinic offerings while keeping the care team informed of patient developments.

Enhancing Quality Metrics: Understand and address the unique challenges and disparities faced by diverse patient populations. Possess knowledge of public health principles and the healthcare delivery system.

Monitoring Health Plan Standards: Review quality measure reports and reach out to patients due for screenings based on their demographic and medical history. Schedule necessary appointments and submit relevant patient information to health plan portals.

Utilizing Health Information Systems: Engage with various Health Information Exchange platforms to facilitate communication and address care gaps effectively.

Data Management: Proficiently use software tools such as Microsoft Excel and Teams to track quality measures and patient care data.

Professional Conduct: Exhibit professionalism in all job functions, including maintaining patient confidentiality, navigating electronic health records, and demonstrating effective problem-solving skills. Work collaboratively within a team environment, showcasing initiative and accountability.

QUALIFICATIONS:

Successful candidates will possess the following:

  • A valid driver's license and personal vehicle insurance compliant with state requirements.
  • Fluency in both English and Spanish.
  • Strong attention to detail and excellent communication skills.
  • Ability to build relationships with patients and healthcare staff.
  • Experience in service coordination and patient navigation.
  • Proficiency in computer applications and office technology.
  • Understanding of healthcare systems and health disparities.
  • Demonstrated organizational skills with the ability to manage multiple tasks effectively.

EDUCATION AND EXPERIENCE:

A high school diploma or GED is required, along with a minimum of three years of experience in health promotion or community service roles.

WORK ENVIRONMENT:

The role may involve exposure to various environmental conditions typical of an office setting, including moderate noise levels and the potential for interacting with individuals in distress.
