Role Summary:
As a valued member of the St. Croix Hospice interdisciplinary care team, the Hospice Aide plays a crucial role in delivering compassionate care to patients. Under the supervision of a Registered Nurse, you will ensure quality, continuity, and safety in patient care, while maintaining accurate documentation.
Key Responsibilities:
- Deliver personal care services including bathing, grooming, dressing, and maintaining hygiene, while ensuring a clean and safe environment.
- Facilitate respectful and confidential communication with patients and their families.
- Monitor and document vital signs, assisting patients with self-administered medications as needed.
- Prepare and serve nutritious meals, including shopping and feeding assistance when required.
- Assess and communicate any changes in the patient's condition to the RN Case Manager.
- Support patients with mobility, exercises, transfers, and positioning to enhance their overall well-being.
