2. **Patient Transition Management:**
- Proactively identify and address potential barriers to patient care progression.
- Collaborate with healthcare teams, community providers, and families to ensure effective transitions of care.
3. **Discharge Planning:**
- Assess clinical stability for discharge and oversee the details of patient transitions.
- Ensure accurate and complete discharge orders, identifying complex discharge issues and coordinating necessary services.
4. **Care System Participation:**
- Engage in processes that prevent avoidable readmissions and facilitate timely referrals.
- Utilize technology to identify patients at risk for readmission and ensure comprehensive clinical handoffs.
Qualifications:
- Bachelor's degree in nursing required.
- 2 to 5 years of nursing experience, including at least 2 years in an acute care environment.
- Current licensure as a Registered Nurse.
- Case/Care Management Certification within 2 years of hire is mandatory.
Physical Requirements:
- Light work, including occasional lifting of up to 20 lbs and frequent lifting of up to 10 lbs.
This role is integral to the delivery of high-quality patient care and requires a dedicated professional committed to enhancing patient outcomes through effective care coordination.
