Job Overview:

Key Responsibilities:
  • Patient Care Coordination:
    • Conduct comprehensive clinical assessments and actively participate in patient care discussions to ensure timely and effective interventions.
    • Identify and address potential barriers to patient care, escalating issues to leadership when necessary.
    • Work collaboratively with healthcare teams, community providers, and families to ensure smooth transitions and continuity of care.
    • Provide support and information regarding palliative care and advanced care planning for patients experiencing chronic conditions.
  • Clinical Transition Management:
    • Coordinate the discharge process for patients in the hospital and Emergency Department, ensuring all clinical details are managed effectively.
    • Serve as a resource for post-discharge care options, ensuring patients and families are well-informed about available services.
    • Assess patients' clinical stability for discharge and ensure all discharge orders are accurate and complete.
  • Readmission Prevention:
    • Engage in transition planning meetings with patients and families to facilitate understanding and preparation for post-discharge care.
    • Utilize technology and tools to identify patients at risk for readmission and ensure comprehensive clinical handoffs.
  • Length of Stay Management:
    • Collaborate with the healthcare team to promote appropriate lengths of stay and timely referrals.
    • Ensure that necessary outpatient services are arranged prior to discharge.
  • Resource Management:
    • Support adherence to clinical guidelines and regulatory requirements during patient care.
    • Work with Social Workers to address any reimbursement or discharge planning concerns.
    • Maintain up-to-date knowledge of payer networks and community resources to coordinate care effectively.
Qualifications:
  • Bachelor's degree in Nursing is required.
  • A minimum of 2 to 5 years of nursing experience, including at least 2 years in an acute care environment.
  • Current RN licensure is required.
  • Certification in Case/Care Management must be obtained within two years of hire.
Physical Requirements:
This position involves light work, including occasional lifting of up to 20 lbs and frequent lifting of up to 10 lbs.
