Job Summary

We are seeking a highly skilled Quality Assurance Nurse to join our team at St Croix Hospice. The successful candidate will be responsible for monitoring, implementing, and evaluating processes and programs to ensure compliance with regulatory, accreditation, and standards of practice.

Key Responsibilities
  1. Quality Assessment and Improvement
  • Implement and monitor the organization's quality assessment performance improvement (QAPI) program.
  • Monitor and measure activities related to QAPI as assigned.
  • Implement quality improvement activities utilizing continuous quality improvement principles and methodologies.
  • Compile, trend, and report quality data in areas such as patient care, safety, risk management, infection control, outcomes, and customer satisfaction.
Safety, Risk Management, and Infection Control
  • Implement processes to monitor and evaluate safety, risk management, and infection control programs.
  • Assist with mandatory in-service programs on OSHA regulations for safety and infection control as requested.
  • Act as a resource to staff in identifying safety and risk management issues for patients and staff.
  • Review incident reports for completeness, risk, standard of care determination, and appropriate follow-up.
Regulatory Compliance
  • Maintain current knowledge related to Medicare Conditions of Participation and the Community Health Accreditation Program's (CHAP) standards.
  • Participate in review of policies and procedures and make recommendations for policy changes.
  • Act as a resource to the clinical team regarding standards, policies, and regulations.
Clinical Resource
  • Maintain updated nursing knowledge and skills.
  • Assist with patient visits as needed.
Community Education
  • Provide assistance to Care Transition Coordinators in meeting community education needs.
  • Maintain positive relationships with referral sources.
  • Maintain a positive image in the community.
Professional Development
  • Maintain professional and technical knowledge by attending educational workshops, reviewing professional publications, establishing personal and professional networks, and participating in professional organizations or societies.
Requirements
  1. Registered nurse with current licensure to practice professional nursing in the state.
  2. Bachelor's degree in Nursing from an accredited program by the National League for Nursing is preferred.
  3. CHPN certification or ability to obtain.
  4. Three to five years of clinical nursing experience, with hospice nursing experience preferred.
  5. Experience with performance improvement, quality assessment, and utilization management.
  6. Self-directed and able to work with minimal supervision.
  7. Excellent coordination and communication skills.
