Job Summary:
The Clinical Care Coordinator is responsible for delivering comprehensive clinical coordination services, which include assessment, planning, and intervention for patients. This role identifies patients through predictive analytics and referrals from healthcare providers to facilitate effective clinical transition planning for patients with complex medical needs. The coordinator may also engage in initial and ongoing evaluations of care levels and manage insurance authorization processes.
Key Position Details:

Tier 1: Requires a minimum of three years of recent RN nursing experience, including at least two years of full-time nursing practice in an acute care environment. A minimum of two years of RNCC experience is necessary.

Tier 2: Requires a minimum of three years of recent RN nursing experience, including at least two years of full-time nursing practice in an acute care environment. A minimum of one year of RNCC experience is necessary.

Tier 3: Requires a minimum of three years of recent RN nursing experience, including at least two years of full-time nursing practice.
  • This is a casual position requiring work every fourth weekend.
  • This role is represented by the MNA.
Job Description:
Principal Responsibilities
  • Facilitates the care progression for patients with complex medical conditions.
    • Conducts clinical assessments and participates in patient care discussions to ensure essential interventions and procedures are executed for optimal patient outcomes.
    • Proactively identifies and resolves potential delays in patient care, escalating issues to leadership as necessary.
    • Collaborates with the healthcare team, community care providers, patients, and families to ensure effective and timely transitions of care.
    • Provides information and support regarding palliative care and advanced care planning for patients facing chronic illnesses.
    • Delivers age-appropriate care tailored to the population served.
  • Coordinates the clinical transition of patients in both hospital and Emergency Department settings.
    • Works with the interdisciplinary team to plan for anticipated transfers or discharges.
    • Acts as a subject matter expert on integrated post-discharge care and services available within the system.
    • Evaluates clinical stability for discharge and oversees the details of transitions.
    • Ensures the accuracy and completeness of discharge orders.
    • Identifies patients and families facing complex discharge challenges and collaborates with providers for follow-up community resources.
    • Conducts screenings or assessments to select patients and communicates the need for follow-up care.
  • Engages in processes aimed at preventing avoidable readmissions.
    • Plans and participates in transition conferences with patients and families.
    • Utilizes tools and technology to identify and intervene with patients at risk for readmission.
    • Ensures comprehensive clinical handoffs for at-risk patients, which may include referrals.
  • Collaborates with the healthcare team to promote appropriate lengths of stay.
    • Employs tools and technology to support effective length of stay management.
    • Facilitates timely referrals and information transfers.
    • Ensures that outpatient complex clinical care services are arranged prior to discharge, along with addressing other complex care needs.
  • Demonstrates effective clinical resource management and compliance with regulatory requirements.
    • Supports level of care activities by utilizing established inpatient guidelines and utilization criteria.
    • Works with Social Workers to identify trends or concerns related to reimbursement and discharge planning.
    • Ensures timely interventions for patients admitted under observation status.
    • Provides information and assistance for identified financial or social needs.
    • Maintains knowledge of government and private payer networks and services.
    • Collaborates with community and healthcare resources to coordinate patient care based on need.
    • Other duties as assigned.
Job Requirements
  • Bachelor's degree in nursing required.
  • 2 to 5 years of nursing experience, including at least two years in an acute care setting required.
  • Licensed Registered Nurse - WI Dept of Safety & Professional Services or Licensed Registered Nurse - MN Board of Nursing required.
  • Case/Care Management Certification within 2 years of hire required. Current employees must complete certification requirements as soon as they meet the necessary work experience.
