Role Summary
As a vital member of the interdisciplinary care team at St. Croix Hospice, the Hospice Aide plays a crucial role in delivering compassionate care to patients in need. Under the supervision of a Registered Nurse, you will ensure quality, continuity, and safety in patient care while maintaining accurate documentation.
Key Responsibilities
- Deliver personal care services including bathing, grooming, dressing, and maintaining hygiene in a clean environment.
- Facilitate respectful and confidential communication with patients and their families.
- Monitor and document vital signs, assisting patients with self-administered medications as required.
- Prepare nutritious meals and assist with feeding when necessary.
- Assess and report any changes in the patient's condition to the RN Case Manager.
- Support patients with mobility, exercise, and therapy activities to enhance their quality of life.
