POSITION SUMMARY

The Clinical Coordinator plays a pivotal role in managing the interdisciplinary Plan of Care for designated patients and their families. This position involves delivering essential interventions and support tailored to the physical, psychological, social, economic, and spiritual needs of patients, families, and caregivers. The Clinical Coordinator is integral to the mission of Arkansas Hospice, Inc., ensuring high-quality care and exceptional service to referring clinicians and partner facilities.

QUALIFICATIONS

Education: High school diploma or GED; completion of a nursing assistant (CNA), licensed practical nurse (LPN), or medical assistant (MA) program is required.

Experience: Prior experience in direct patient care within a hospital, long-term care facility, physician's office, or hospice setting is preferred.

Licensure: Certification as a Nursing Assistant (CNA), Licensed Practical Nurse (LPN), or Medical Assistant (MA) is necessary.

Additional Requirements: Eligibility for a Letter of Determination and successful completion of a skills evaluation test. A valid driver's license and car insurance are required. Flexibility in working hours and occasional overtime may be necessary. CPR certification is mandatory.

PRIMARY RESPONSIBILITIES

  1. Ensure safe, high-quality, and comprehensive care coordination for assigned patients, families, and caregivers; oversee the development and updates of the patient's Plan of Care.
  2. Conduct ongoing assessments of physical and emotional needs of patients and families; provide clear information regarding services and care expectations.
  3. Evaluate patient and family needs continuously and coordinate necessary services (DME, home health, hospice) throughout the patient's care journey.
  4. Collaborate with the interdisciplinary team to facilitate communication and coordination from referral through care transitions associated with serious illness.
  5. Gather data on physical, psychological, social, and spiritual factors affecting patient and family health; integrate this data into care plans and reports.
  6. Promote effective communication with patients, families, team members, and other healthcare providers; initiate contact with providers and community resources to report changes and coordinate care.
  7. Maintain accurate patient records reflecting problems, plans, actions, and goals, ensuring updates are documented as they occur.
  8. Act as a liaison between partner facilities and Arkansas Hospice, Inc.
  9. Document care provided daily in accordance with organizational standards.
  10. Recognize and report any changes in the patient's condition or family situation to the appropriate providers.
  11. Utilize resources effectively and teach families and caregivers proper patient care techniques.
  12. Conduct initial and follow-up visits to patients in various settings, including homes and hospitals, either in person or via telehealth.
  13. Participate actively in interdisciplinary team meetings, case conferences, and care plan discussions.
  14. Maintain precise documentation of patient assignments as directed by providers.
  15. Engage in Quality Assurance and Performance Improvement (QAPI) initiatives.
  16. Uphold confidentiality at all times.
  17. Exhibit excellent customer service skills; seek opportunities to assist patients, families, and colleagues; demonstrate teamwork and cooperation.
  18. Follow supervisor's directions and seek additional information as needed; work independently with minimal supervision.
  19. Adhere to all organizational and departmental policies and procedures.
  20. Meet organizational standards for attendance and punctuality; notify supervisors in a timely manner regarding absences or lateness.
  21. Attend all required meetings and in-services; pursue opportunities for professional development as appropriate.
  22. Perform other assigned duties as necessary.
