POSITION SUMMARY

The Access Intake Specialist is responsible for managing incoming calls related to hospice care referrals, processing these referrals, and collaborating with Access Clinicians to ensure prompt responses to referral inquiries. This role is essential in facilitating the transition of referrals from the Access team to the designated hospice interdisciplinary team.

QUALIFICATIONS

Education: High school diploma or equivalent.

Experience: At least one year of experience in a healthcare setting such as a hospital, home health agency, hospice, or similar environment.

Additional Requirements: Must possess a valid driver's license, vehicle, and insurance.

PRIMARY RESPONSIBILITIES

  1. Welcomes callers, addresses general inquiries, records patient details, and logs all referral calls into the electronic medical record system.
  2. Provides guidance and assistance to patients, families, and referral sources regarding potential hospice care options.
  3. Adheres to the established referral intake protocol when processing referrals.
  4. Communicates relevant referral information and scheduled appointments to the appropriate Access Clinician, coordinating with the Access Coordinator or Supervisor as necessary.
  5. Maintains effective communication with patients, families, interdisciplinary team members, and other healthcare professionals.
  6. Ensures timely delivery of all referral information to the designated Access Clinician and team.
  7. Maintains organized records pertinent to the access team in a designated, accessible location.
  8. Demonstrates understanding of the policies of Arkansas Hospice, Inc. and relevant state and federal regulations that guide patient eligibility for hospice services.
  9. Interacts professionally with supervisors, patients, family members, and all stakeholders.
  10. Utilizes time and resources efficiently to maximize productivity.
  11. Engages in Performance Improvement initiatives.
  12. Upholds confidentiality at all times.
  13. Accepts guidance and adheres to instructions from supervisors.
  14. Complies with all organizational and departmental policies and procedures.
  15. Maintains consistent attendance and punctuality; informs supervisors promptly of any absences or delays.
  16. Participates in required meetings and training sessions; seeks opportunities for further professional development.
  17. Performs additional duties as assigned.
