POSITION SUMMARY
As a Clinical Care Navigator, your role will involve facilitating care for patients with dementia through the (Guiding an Improved Dementia Experience) GUIDE model. You will foster strong, collaborative relationships with both internal teams and external partners to guarantee that patients receive suitable and high-quality care. Key responsibilities include assessment, care planning, coordination of services, and transitional care management. This position entails conducting in-home evaluations of beneficiaries and frequent telephonic or video consultations to provide assistance to patients and their caregivers.
QUALIFICATIONS
Education: High school diploma or GED; completion of a Medical Assistant, Certified Nursing Assistant, or Licensed Practical Nurse training program that meets Arkansas Department of Health standards is required.
Experience: A minimum of two (2) years of experience in healthcare, with preference given to candidates who have worked with geriatric or dementia populations.
Licensure: Certified Nursing Assistant (CNA) certification.
Additional Requirements: Eligibility for a Letter of Determination and successful completion of a skills evaluation test. A valid Arkansas driver's license, car insurance, and access to a vehicle are also necessary.
PRIMARY RESPONSIBILITIES
- Conduct comprehensive assessments covering clinical, behavioral, psychosocial, and advance care planning aspects.
- Evaluate current health needs, set goals, and create personalized care plans.
- Develop and maintain resource directories and network with other service providers to assist clients.
- Familiarity with the rules and regulations of the GUIDE Program.
- Regularly review the GUIDE program website to ensure all information regarding beneficiaries, partners, and providers is accurate and current.
- Submit detailed and timely documentation on the same day services are delivered.
- Educate patients and families about available resources and services.
- Facilitate support groups for patients and their caregivers.
- Collaborate with primary care clinicians to ensure high-quality, team-based care.
- Utilize management tools to support appropriate transitional care management.
- Work with hospitals, rehabilitation centers, and skilled nursing facilities to manage patient inpatient stays and discharge plans.
- Track necessary data and complete benchmarking tools.
- Communicate effectively with both internal and external stakeholders.
- Assist in reducing unnecessary emergency department visits to minimize costs.
- Enhance care coordination for a complex geriatric population.
- Maintain proficiency in community resources.
- Regularly monitor Remote Patient Monitoring (RPM) software and alert clinicians to any abnormal readings.
- Assist clinicians in educating patients on the use of RPM equipment.
- Coordinate with billing to ensure timely submission of all relevant RPM and GUIDE codes.
- Engage proactively with providers to identify high-risk patients.
- Ensure compliance with all applicable state and federal laws, regulatory standards, contracts, and organizational policies.
- Accept guidance and follow instructions from supervisors; seek additional information as needed; work with minimal supervision.
- Complete special projects as assigned by management.
- Adhere to all organizational and departmental policies and procedures.
- Maintain organizational standards for attendance and punctuality; notify supervisors in a timely manner of any absences or delays.
