Summary:

The Patient Care Coordinator collaborates with case managers to ensure seamless continuity of care for patients transitioning to home upon discharge.

This role involves making referrals to community support services and effectively communicating these arrangements with the case manager while documenting all relevant information in the patient's medical record.

Additional responsibilities include assisting in the preparation of appeal letters for denied medical stays and conducting follow-up phone calls post-discharge.

  • Facilitate referrals to community resource agencies for patients with home discharge needs.
  • Coordinate and communicate discharge planning activities effectively.
  • Document the agencies providing services in the medical record, along with the completion of follow-up calls.
  • Conduct discharge follow-up calls, documenting findings in the medical record, and communicate any identified gaps in care.
  • Assist the case manager in drafting appeal letters for denied inpatient admissions.
