The Patient Care Coordinator plays a crucial role in ensuring the seamless integration of medical and social services for patients during their hospital stay. This full-time position requires minimal on-call duties and is focused on enhancing patient outcomes through effective case management.
Key Responsibilities:
- Conduct comprehensive assessments of patient and family needs upon admission and throughout the hospital stay.
- Evaluate the medical necessity for inpatient care and facilitate communication with insurance providers and key hospital personnel.
- Collaborate with licensed independent practitioners (LIPs) and clinical staff to develop and implement effective discharge plans.
- Advocate for Patient and Family Centered care, ensuring a safe and supportive discharge process.
- Monitor patient progress and identify potential discharge needs and community resources early in the hospitalization.
- Coordinate communication with third-party payors, medical staff, families, and patients regarding care plans, length of stay, and home care arrangements.
- Maintain regular communication with physicians to foster effective relationships and ensure continuity of care.
- Document assessments, problem identification, desired outcomes, planning, implementation, and evaluation to ensure comprehensive care delivery.
- Facilitate referrals to community resource agencies for patients requiring home discharge support and conduct follow-up calls post-discharge.
- Uphold exceptional customer service standards and support the organization’s commitment to Family Centered Care.
- Perform additional duties as assigned.
Qualifications:
Required Education:
Associate's degree or equivalent from a two-year college program or technical school, or nursing diploma.
Recommended Education:
Bachelor's degree in Nursing or a related field.
Required Work Experience:
Minimum of three years of nursing experience.
Required Certifications:
Accredited Case Manager (ACM) and/or Certified Case Manager (CCM) certifications are required within one year of eligibility. An active RN License in Arkansas or a Compact State is also necessary.
