Position Overview:
The Inpatient Care Coordination Specialist plays a crucial role in enhancing the recovery experiences of patients. This position involves conducting regular functional evaluations and collaborating with the interdisciplinary care team to utilize proprietary tools for effective discharge planning. The Specialist engages with patients and their families to provide essential information and facilitate informed health care decisions, ensuring seamless transitions between care settings.

Key Responsibilities:

  • Conduct comprehensive assessments of patients in Skilled Nursing Facilities (SNF) using clinical expertise and appropriate evaluation tools.
  • Collaborate with health care providers and families to review discharge plans and set targets for Length of Stay (LOS) and outcomes.
  • Perform concurrent reviews and manage authorizations efficiently.
  • Work closely with health care teams, including physicians and social workers, to optimize discharge processes.
  • Monitor patient progress towards discharge goals and address any barriers to successful transitions.
  • Participate in interdisciplinary rounds to provide updates and insights to management.
  • Ensure appropriate referrals to health plans and community services are made.
  • Engage with patients and families regularly, both remotely and in person, to support their care needs.
  • Attend care conferences to discuss patient progress and needs.
  • Evaluate ongoing appropriateness for SNF placement based on established criteria.
  • Coordinate peer reviews and manage documentation in compliance with organizational standards.
  • Utilize time management skills to effectively handle assigned caseloads and assist colleagues as needed.
  • Maintain confidentiality and adhere to all regulatory requirements regarding patient information.

Qualifications:
Required:
  • Active, unrestricted clinical license (Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist).
  • Minimum of 3 years of clinical experience.
  • Proficiency in Microsoft Office Suite (Outlook, Excel, Word).
Preferred:
  • Experience in Case Management (2+ years).
  • Background in geriatric care or home health nursing.
  • Strong communication and problem-solving skills.
  • Ability to prioritize and manage multiple tasks effectively.
