Community Support Coordinator

The Community Support Coordinator plays a vital role in delivering a range of case management and housing support services to individuals residing in our facilities, aiding their transition and integration into both the residence and the wider community. This position ensures the effective execution of outreach initiatives and case management tasks to achieve program objectives.

The Community Support Coordinator is also tasked with conducting regular assessments of service needs for participants and formulating collaborative service plans on a quarterly basis to address identified requirements and aspirations. It is essential to maintain consistent personal contact with clients, fostering trusting relationships to monitor their physical and emotional readiness for independent living, as well as their need for additional community services.

Qualifications:

  • A minimum of one year of experience in mental health with the target demographic; experience in case management and/or residential programs is essential.
  • At least 60 college credits are required; an Associate's or Bachelor's degree in social welfare, behavioral health, or a related discipline is preferred.
  • A valid driver's license in good standing is necessary.
  • Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint) and familiarity with database navigation is required.
